Non-compliance follow-up·December 8, 2023

On 12/8/2023 at 10:20am, Licensing Program Analyst (LPA) Andrea Pittman conducted a Case Management Legal/Non-Compliance Inspection at the facility and was met by the Facility Representative. The LPA disclosed the purpose of the inspection and was permitted entry by the Facility Representative. The Director Debora Barajas later met with LPA Pittman and guided the LPA on a tour of the facility. Upon entry to the facility, the LPA observed 22 children in care and six staff providing care and supervision. During the inspection, LPA Pittman explained the details of the Immediate Exclusion Order for Michelle Parks. LPA Pittman observed that the employee, Michele Parks, was not present. The order to immediately exclude Michele Parks was discussed in detail with the Director. They are aware that Michele Parks must not be physically present in the facility nor can they have contact with children in care. The Director acknowledges the receipt of the Immediate Exclusion Order served today, Friday, December 8 th , 2023. The California Department of Social Services (CDSS) has determined that Michele Parks continued or future contact with clients or presence in any community care facility, child care facility, residential care facility for the elderly, or any other facility licensed by the CDSS constitutes a threat to the health, welfare, or safety of the clients in care. Upon receipt of the immediate exclusion order, Michele Parks, must remove themselves from any contact with clients and not be physically present in any state licensed facilities. This action is final until Michele Parks is notified otherwise, in writing, by the CDSS. An exit interview was conducted, a copy of this Report, a Notice of Site visit, and Appeal rights were provided and discussed with the Director.
