What the inspector wrote

On December 17, 2024, at 10:10 a.m., Licensing Program Analyst (LPA) Zirbes met with Facility Representative Sherry Rice (Kaufman) for the purpose of conducting a case management - Annual continuation inspection for the Preschool Center. During the inspection, LPA observed 6 toddlers with one teacher and one aide providing supervision and 23 preschool age children with five teachers, two aides providing supervision. Additional Administrative and support staff were available . Based on LPA observation the Center was meeting the ratio and capacity regulations during this inspection. During this inspection, LPA reviewed five child files and five personnel records. Based on LPA review, one of five child files reviewed did not contain documentation of the completed medical assessment. Per Administrator, child 1 (C1) completed the medical assessment however the paperwork was not uploaded into the electronic file and could not be located during this inspection. Based on LPA review, one technical violation was issued as C1s files was missing documentation of the medical assessment. Per LPA record review, four of five staff files (S1, S2, S3 and S4) did not complete the two year renewal of the mandated reporter training as required. Per Administrator, the Center was in the process of updating all mandated reporter training and had already advised the staff members of the need to complete the training. However staff members did not complete the training's as of this date. This is a potential risk therefore a type B citation was issued, refer to LIC 809D. In addition, per LPA review, two of five staff files (S3 and S4) did not contain documentation of the required immunization's (Tdap, Measles). Administrator reviewed the file and confirmed the immunization's paperwork was missing. This is a potential risk therefore a type B citation was issued. Refer to LIC 809D. Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
