What Happened

During a complaint investigation, LPAs learned the facility had an ongoing hand, foot, and mouth disease outbreak. The director acknowledged the outbreak had not been reported to Licensing within the required 24-hour timeframe, and a reporting violation was issued.

Reporting Requirements: a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event.Epidemic outbreaks. This requirement is not met as evidenced by:Based on information recieved from the Director, there were apoximently 3 children with a confirmed case of the disease.This poses a potential health, safety or personal rights risk to persons in care.

101212 Reporting Requirements(f) The items specified in (d)(1)(A) through (H) above shall also be reported to the child's authorized representative.The requirement was not met as evidenced by: Based on interviews conducted and the Director's own admission, the Licensee did not notify parents/authorized representatives of the Hands-Foot-and-Mouth Disease outbreak that occurred at the facility on or around 04/08/2022. This poses an immediate health, safety or personal rights risk to persons in care.
