What the inspector wrote

A case management visit is being conducted in response to the receipt of an unusual incident report (UIR) from the facility. It indicates on 4/4/2025 two children, C1 and C2 were pulling on each other when C2 deciding to push instead of pull. C1 then fell down off of a chair that they were sitting on an appeared to hurt their ankle. Facility records were reviewed and S1, S2, C1, and C2, were interviewed. The information gathered stated the incident happened within a few seconds, the staff were quick to respond to the incident, and staff were in the room when the incident happened. Information gathered stated that no child was seriously injured during the event. The information gathered stated that none of the facility's staff observed the event between C1 and C2. Based on the information gathered, the following violations have been identified: the facility did not provide visual observation to all children at all times. SEE LIC809D for cited deficiencies of the California Code of Regulations, Title 22, Div. 12. Exit interview conducted and report was reviewed with the Director Charlene Bunnell-McAlister . A notice of site visit was given to Director Charlene Bunnell-McAlister and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. This report must be made available to the public for 3 years. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. An exit interview was conducted, appeal rights were discussed, and a copy of this report was provided to facility staff.
