What the inspector wrote

Based upon the information gathered in regards to the flea/pest infestation, the preponderance of evidence standard has been met, and therefore, the above allegations are found to be SUBSTANTIATED . See LIC 9099D for deficiency cited. An exit interview was conducted, and a copy of this report was reviewed and provided to the Director, Charlene Bunnell-Mcalister. Appeal rights were discussed and provided during the exit interview. A notice of site visit was given and must remain posted for 30 days. Licensee must have parents of all current and any newly enrolled clients within the next 12 months, complete the Parent Notification Requirements form LIC 9224, and place the completed form in the child’s facility file. A civil penalty of $100 per violation will be assessed for noncompliance LPA advised Director that the center should be also following the regulations set forth by the Department of Pesticide.

This poses an immediate health, safety or personal rights risk to persons in care.

    Personal Rights: The licensee shall ensure that each child is accorded the following personal rights:To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs.The requirement was not met as evidenced by: Based on interviews conducted, the infants in care had multiple flea bites on various parts of the body. Facility failed to provide a safe environment for the children in care.This poses an immediate health, safety or personal rights risk to persons in care.

  • BUILDINGS AND GROUNDS

    Buildings and GroundsThe child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. The licensee shall take measures to keep the center free of flies, other insects, and rodents.The requirement was not met as evidenced by: Based on interviews conducted and observation made by the LPA. The facility is not ensuring the children's safety, appropriate measures have not been taken to alleviate the insect infestation.This poses an immediate health, safety or personal rights risk to persons in care.
