What the inspector wrote

On 11/07/2025, Licensing Program Analyst (LPA) Raymond Moorehead arrived at the facility to conduct a case management visit with Jennifer Davidson, Head of School. A case management visit is being conducted in response to the receipt of an Unusual Incident Report (UIR) from the facility. The UIR was received by the licensing agency on 10/31/2025 as the incident was self-reported. During today's visit, LPA investigated the incident. LPA conducted staff interviews, collected pertinent documentation, and made observations. Please note that a tour/census of the facility was taken. The following was disclosed. The facility reported that two children teamed up and were able to sneak out of their classroom, and were able to leave the facility. Further, it was reported that the two subject children were able to walk across the street and walk up the street. It was also reported that two individuals observed the two subject children that were running from the facility and called the facility immediately. It was reported that the two subject children were safely retrieved and brought back to the facility by administrative staff members. The facility was transparent with the incident and showed LPA exactly where the incident occurred. Pertinent interviews disclosed that the two subject children were able to sneak outside of their bathroom's back door, into the classroom back patio, and then to the front of the facility. It was observed that the front of the facility and parking lot is surrounded by a gate. However, it was stated that surveillance video footage showed the subject children running out the gate, as a car had just went through it. It was stated that the gate was open for a very limited amount of time. Continued on LIC 809-C. Based on interviews conducted, documentation reviewed, and observations made, it was revealed that the facility failed to provide adequate supervision for the two subject children, due to them being to leave the facility. This is in violation of Title 22 Regulation - 101229 Responsibility for Providing Care and Supervision which states the following: (a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. See LIC 9099-D for cited deficiency. A immediate civil penalty of $500.00 was assessed, due to an Absence of Supervision. Due to the seriousness of the violation observed, LPA conducted a consultation with the Head of School regarding staff training requirements related to providing adequate Supervision. Head of school agreed to implement corrective action, including re-training all staff on supervision expectations. Head of School also stated that the facility has already implemented the following actions: 1. Securing of all possible exits for children 2. Walk through of each class to identify any gaps 3. Going over Supervision expectations with each teacher in person and in writing LPA informed Licensee that this report dated 11/07/2025 documents 1 Type A citation, which shall be posted for 30 consecutive days as there was an immediate risk to the health, safety, and personal rights of children in care. Also, LPA informed the facility representative to provide a copy of this licensing report dated 11/07/2025 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. A Notice of Site Visit was provided and must remain posted for 30 days. Failure to maintain posting as required will result in a civil penalty of $100.00. An exit interview was conducted, and the report was reviewed with Head of School Jennifer Davidson.
