What the inspector wrote

On 12/11/2025, at 11:00 AM, an informal conference was held at the Riverside Regional Office. Present at the conference were Regional Manager Stephanie Hudak, Licensing Program Manager (LPM) Aaron Ross, Licensing Program Analyst (LPA) Raymond Moorehead, Head of School Jennifer Davidson, and Staff Coordinator Merel Adriano. The purpose of the meeting was to review and discuss the following: 1.) Supervision Please see Licensing case management report dated on 11/07/2025. The purpose of the informal conference was to review the circumstances that led to the reported incident, discuss regulatory responsibilities, and discuss corrective actions that were implemented to prevent recurrence. Facility leadership and staff were present for the conference. The incident was reviewed in detail, including the unlocked bathroom's back door leading to the classroom's back patio, the malfunctioning gate leading to the front parking lot, and the lapse in supervision that allowed the two children to leave the facility undetected. During the informal conference, the facility outlined several corrective measures that have been implemented since the incident. These include installing a high placed lock on the bathroom door leading to the classroom's back patio, reinforcing the back patio's gate with a new lock and latch system, repairing the gate leading to the parking lot, adjusting staff schedules to improve supervision coverage, and updating internal procedures for monitoring bathroom use. The facility acknowledged the seriousness of the incident and affirmed its commitment to ensuring the health and safety of children in care. RM, LPM, and LPA discussed expectations moving forward, including ongoing supervision practices, maintaining secure exits, and monitoring the effectiveness of newly implemented safety measures. The facility also reported long-term plans to remove the bathroom patio door entirely, pending consultation with the fire department. The facility was also suggested to conduct a new fire clearance consultation, to ensure compliance with newly installed door locks. Facility Representatives acknowledged their responsibilities in protecting the health and safety of all children in care. Facility Representatives both understand the seriousness around the topics that were discussed during today's meeting. As a result of this informal conference, Facility Representatives understands the Department's expectations and agrees to maintain compliance with Title 22 Regulations. Exit interview was conducted and the report was reviewed with Head of School Jennifer Davidson, Staff Coordinator Merel Adriano. A copy of this report was provided to Facility Representatives.
