What the inspector wrote

On 03/16/2023 at 2:11 PM, A required \342\200\223 1 year inspection is being conducted as part of a compliance review. Licensing Program Analyst (LPA) Susan Brewer, was greeted and granted entry by Head of School Jennifer Davidson and Director Kim Montague, toured the school-age center, inside and out. The following was observed: The School Age Program is located on in the C Building, Room C3-Lower Elementary and C9-Upper Elementary, Ages 6 years old through 12 years old. Operation Days and Hours: Monday to Friday; 3:30 PM to 5:00 PM. During School Holidays and Summer Break Days and Hours are Monday through Friday 7:45 AM to 5:00 PM. The inspection consisted of reviews of the following domains: Food Service Reporting Requirements Physical Plant, Care and Supervision Children Records, Staff Records Staffing Ratio and Capacity Personal Rights. The inspection found the facility to be in compliance in these domains, except as noted on the LIC809-D A review of the staff records and children's records were conducted as part of this evaluation. The licensee is asked to update the following documents and submit to licensing within 30 days on or before 04/16/2023: Only if changes have been made: 1. LIC 500 Personnel Report 2. LIC 610 Emergency & Disaster Plan 3. Parent Handbook/ Program Curriculum/Admission policies and procedures/ fee schedule (only if changes have been made) 4. LIC 309 Administrative Organization 5. LIC 308 Designation of Administrative Responsibility The following items have been posted and are updated where necessary: License, Emergency Disaster Plan (LIC610) and Earthquake Preparedness Checklist (LIC9148); Parent\342\200\231s Rights Poster (PUB393), Personal Rights (LIC613A); Child Car Seat Law, Menu \302\267 The facility is operating within the terms of the license on 03/16/2023. \302\267 Facility is clean, safe and sanitary and in good repair on 03/16/2023. \302\267 Ratios were met during this inspection: A census of 28 children, supervised by 5 staff. \302\267 Appropriate supervision was provided during this inspection on 03/16/2023. \302\267 Classrooms are equipped with age appropriate furniture and equipment in good condition. \302\267 Classrooms are clean and free of hazards. \302\267 Fire extinguishers are in the green, smoke alarms are hardwired and carbon monoxide devices were tested on 03/16/2023. \302\267 All materials and surface accessible to children are toxic free. \302\267 No weapons stored at the facility on 03/16/2023. \302\267 There are no bodies of water present on 03/16/2023. All wading pools or similar product must be emptied immediately after use and stored in an upright position. \302\267 Medications are stored where inaccessible to children in the classroom locked cabinet, office and refrigerated in staff kitchen with is inaccessible to children in care. \302\267 Center is equipped to isolate and care for any child who becomes ill during the day at the front reception area or director\342\200\231s office. \302\267 Hazards are stored where inaccessible to children which include: disinfectants, cleaning solutions and other items that are dangerous to children on 03/16/2023. \302\267 All floors shall be clean and safe on 03/16/2023. \302\267 Measures are taken to keep the facility free of flies, other insects and rodents on a monthly basis. \302\267 Toxins are locked on 03/16/2023. \302\267 Bathrooms were observed to be safe, sanitary and in operating condition. \302\267 Playgrounds are enclosed by appropriate fences and free of hazards. \302\267 Outdoor activity areas are supplied with age and size appropriate equipment in good condition \302\267 Food preparation area is clean and free of vermin on 03/16/2023. \302\267 Food is stored appropriately and protected from contamination. \302\267 All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. \302\267 Uncontaminated drinking water shall be readily available both indoors and out and is provided by pitchers of filtered water and individual cups. There are water fountains, used as well. \302\267 The areas around or under high climbing equipment, swings, slides, and similar equipment shall be cushioned with material that absorbs a fall wood chips, natural grass and sand. \302\267 Menus shall be posted at least one week in advance in a place visible by the child\342\200\231s authorized representative, dated and kept on file for 30 days, and made available upon request. Only snacks are provided to children in the afterschool program. \302\267 Sign in/Sign out record was reviewed and meets regulation requirements. The facility utilizes the School Pass Software program for Electronic Sign-in/outs. Parents have access to the phone applications to sign children in and out of daycare and report absences. \302\267 A Staff member is present with current Pediatric CPR/First Aid which expires on 08/2023. \302\267 Opening and closing staff member\342\200\231s CPR/First Aid expires on 08/2024 \302\267 Director completed Health and Safety Training: NOT ON FILE. \302\267 Staff qualifications and files were reviewed \342\200\223 health screening is on file and all staff meet Educational requirements, however 2 of 3 staff DO NOT MEET health requirements for performing assigned tasks. LPA Susan Brewer conducted a review of staff records which revealed that 2 of Staff 5 staff present are missing verification of MMR, and 1 of 5 staff present is missing verification of Flu or letter to decline. Staff present were missing the Mandated Reporter Training Certificates. \302\267 Staff received on the job training for house keeping, sanitation and universal health precautions \302\267 Each child\342\200\231s file is complete on 03/16/2023. \302\267 Licensee was informed of the Department has inspection authority per Health and Safety Codes. \302\267 Documentation of fire & earthquake drills to be conducted every six months: Last drill on Last Fire drill on 12/01/2022, at 10:00 AM, lasted 15 minutes, with 58 teachers, 350 students as a combination center. \302\267 Documentation of Lead Testing is on file: Report dated 02/22/2023 indicates no ALE detected. \302\267 A review of staff records on 03/13/2023 indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. The Licensee was informed of their reporting requirements and is provided with the Regional Office\342\200\231s Unusual Incident Reporting email mailbox: UnusualIncidentReportsDO09@dss.ca.gov This facility provides Incidental Medical Services \342\200\223 IMS. LPA reviewed storage of medication area and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. No children are receiving IMS at this time. Director Kim Montague and Jennifer Davidson, were reminded that all adults 18 and over working in the center, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. The Licensee can submit transfer forms to associate new individuals or to disassociate someone from your facility at: Associations_Disassociations862@dss.ca.gov for Riverside Regional Office. LPA Susan Brewer, informed Jennifer David son, of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
