What the inspector wrote

On October 8, 2025, Licensing Program Analyst (LPA) Calloway, made an unannounced case management inspection to the above facility. The purpose of the case management was to follow up on a self-reported Unusual Incident Report (UIR) submitted to the Palmdale Regional Office on October 3, 2025. LPA toured the facility with the representative and observed forty-one (41)-day care children and eight staff in active care. On 10/3/25, Child 1 (C1) was observed by Staff climbing on wooden baby crate. When C1 attempted to climb off the crate C1 lost their balance and fell hitting their head on the concrete. C1 sustained an injury to the back right side of the head. LPA conducted interviews with all relevant parties involved. LPA reviewed staff and children files and completed a safety inspection of the playground and observed the playground to be safe and free of debris. Based on observation and interviews and record review, it was determined that C1 was provided supervision while on the playground and although C1 fell, it could not be prevented. Staff followed the proper protocol by rendering aid to the child, notifying C1\342\200\231s parent, and notified Community Care Licensing (CCL) as soon as the injury occurred. There are no deficiencies cited during this inspection. Exit interview was conducted and a copy of this report was read, a Notice of Site Visit, and was provided to Magdalena Lozano, Representative at the facility. A Notice of Site Visit must remain posted for thirty (30) consecutive days. Failure to maintain posting will result in a $100 civil penalty.
