What the inspector wrote

On November 19, 2025, Licensing Program Analyst (LPA) Calloway, made an unannounced case management inspection to the above facility. The purpose of the case management was to follow up on a self-reported Unusual Incident Report (UIR) submitted to the Palmdale Regional Office on November 14, 2025. LPA toured the facility with the representative and observed eight-day care children and two staff in active care. All adults 18 years and over have Criminal Record Clearance (DOJ) and Child Abuse Central Index Clearance and are associated with the facility. The reported information was on November 14, 2025, Staff observed a Child 1 (C1) during outside time sitting on a bench; C1 fell backwards hitting their head and causing a \302\275 inch red mark. LPA conducted interviews with all relevant parties involved. LPA reviewed the incident report and completed a safety inspection of the playground. Based on observation of the playground the activities tables were in close correlation with each other with artificial turf underneath a cushioned surface. LPA was not able to view surveillance footage during this inspection. Further investigation may be needed. Based on interviews and record review, it was determined that C1 was trying to stand up and take their legs from under the table, lost their balance, and fell backwards hitting the bench behind them. One Staff was sitting at the table nearby and observed the fall but was unable to prevent the injury from occurring. The protocol was followed during the incident. The staff notified C1\342\200\231s parents and Community Care Licensing (CCL) and conducted interviews with their staff. There are no deficiencies cited during this inspection. Exit interview was conducted and a copy of this report was read, this report, and a Notice of Site Visit, were provided to Magdalena Lozano, Representative at the facility. A Notice of Site Visit must remain posted for thirty (30) consecutive days. Failure to maintain posting will result in a $100 civil penalty.
