Licensing Program Analyst (LPA) Susan Brewer, arrived at the facility to discuss and provide a notification letter of Order to Licensee of Immediate Exclusion from All Licensed Facilities for Staff Member, SHAVONNE GLADNEY. The letter states in part, that as the result of an investigation by the California Department of Social Services of an allegation of conduct that is inimical and personal rights against SHAVAONNE GLADNEY. The California Department of Social Services (CDSS) has determined that the continued or future contact with clients or presence of this person in the child day care facility, constitutes a threat to the health, morals, welfare or safety of the clients in care. The letter states that upon receipt of the notice, SHAVONNE GLADNEY must remove themselves from any contact with clients and not be physically present in any facility. This action is final until you are notified otherwise, in writing by CDSS. An appeal can be made against this decision in writing to Department of Social Services - Community Care Licensing - Attention: Employee Exclusion Hearing - 744 P Street, MS T-9-15-48 Sacramento, CA 95814. The appeal must be received within 15 days of the date of this letter. The letter is dated January 18, 2023. Facility Director Daniela Varga, stated that SHAVONNE GLADNEY, does not work at the facility. The Director stated they will provide a copy of the Immediate Exclusion notice to the licensee Preschool Services Department (PSD) South Redlands Headstart/State Preschool. LPA confirmed that SHAVONNE GLADNEY was not present during this visit. LPA Susan Brewer, reviewed the Immediate Exclusion Orders with Director Daniela Varga. A copy of the letter was provided to Director Daniela Varga, on this date 02/07/2023, and stated that they understand the Order to Individual for Immediate Exclusion from All Facilities for SHAVONNE GLADNEY. An exit interview was conducted and a copy of this report was provided to Director Daniela Varga, on this date 02/07/2023. A copy of this report must be made available to the public for three years.
