On date and time listed above, Licensing Program Analyst (LPA) Raymond Moorehead arrived at the facility to close out a Case Management investigation, regarding a self-reported incident. LPA conducted a visit for the purpose of closing out an incident report investigation. Please note that this investigation was based off of a self-reported Unusual Incident Report (UIR) made by the facility and was initiated on 03/17/2026. The report involved an allegation that a staff member used inappropriate physical handling while redirecting a child following an incident in which the child struck another child. Throughout the course of the investigation, LPA conducted interviews with pertinent individuals, collected relevant documentation, and made observations. The subject staff member denied the allegation. Interviews conducted with the subject children were unable to corroborate the reported incident. Further, it was determined that there were no additional staff members who witnessed the incident at the time it reportedly occurred. Based on the information obtained during the investigation, there was insufficient corroborating evidence to support the allegation. Therefore, no citations were issued during today's visit. LPA conducted an exit interview with Acting Supervisor Neven Gabra, and provided a copy of this report. A Notice of Site Visit was issued and must remain posted for the next 30 days.
