What the inspector wrote

On this date and time, Licensing Program Analysts (LPAs) Laura Mejorado and Chase Atherton arrived at the facility to conduct a case management visit in response to the receipt of an unusual incident report (UIR). The UIR was received by the Riverside Child Care Regional office, on 10/23/25. The UIR documented an incident of a child sustaining an injury while in care. Upon arrival, LPAs met with Acting Site Supervisor Rosario Pulido-Zavala and stated the purpose of the visit. Records were reviewed and interviews were conducted. It was reported on 10/22/25 two children were running on the playground when they accidentally ran into each other. The subject child hit their forehead causing an open wound. Staff observed the incident and provided first aid by cleaning the area and applying gauze. The paramedics were called and the subject child's authorized representative opted to take the child to the emergency room where the subject child received stitches. The subject child has not returned since the incident but is expected to return once the wound heals. Based on the information gathered and compiled during this visit no citations were issued, at this time. Exit interview conducted and report was reviewed with Acting Site Supervisor Rosario Pulido-Zavals. A notice of site visit was given and must remain posted for 30 days. A copy of this report must be made available to the public, at the facility site, for 3 years.
