Inspector’s narrative

What the inspector wrote

It was reported, on multiple occasions, a child has been left in a soiled diaper during their time at the facility resulting in diaper rashes. It was reported; the child has gone home in the same diaper they were dropped off in. Facility policy is to check diapers every hour and document diaper changes on the facility app. While conducting interviews it was disclosed there were incidents of a child being left in the same diaper or not receiving a diaper change brought to the facilities attention. The facility then implemented a new documentation policy for diapers, however, since implementing the new policy another incident occurred. Interviews disclosed that the most recent incident occurred on 9/4/25, staff were unsure if the child had received a diaper change and stated while not intentional, they may have forgotten. While reviewing records, a diaper change was not document for the child in the facility app for 9/4/25. Based on LPA observations, interviews, and record review, the preponderance of evidence standard has been met, therefore the above allegation(s) is found to be SUBSTANTIATED. California Code of Regulations, (Title 22, Division 12 & Chapter 1), are being cited on the attached LIC9099D. LPA Mejorado informed facility representative Debra Rivas that this report dated 9/10/25 document(s) one Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Also, LPA Mejorado informed facility representative Debra Rivas to provide a copy of this licensing report dated 9/10/25 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview conducted and report was reviewed with the facility representative Debra Rivas.
