Inspector’s narrative

What the inspector wrote

It was alleged a child’s records were unable to be located at the facility. Interviews with pertinent parties stated children’s records are stored for 10 years. It was disclosed there was a recent incident where a child’s records were unable to be located during drop off. The child had previously attended the year prior and was found in the facility database under withdrawn children; however, facility conducted a search of currently enrolled and withdrawn children’s records and was unable to locate the file. Based on LPA interviews and record’s review, the preponderance of evidence standard has been met, therefore the above allegation(s) is found to be SUBSTANTIATED. California Code of Regulations, (Title 22, Division 12 & Chapter 1), are being cited on the attached LIC9099D. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with Director Debra Rivas.
