What the inspector wrote

On this date and time, Licensing Program Analyst (LPA), Laura Mejorado conducted an annual inspection as part of a compliance review. LPA met with Lead Teacher Mayra Mares. Later in the inspection LPA met with Facility Representative Alexis Bogarin, Early Education Administrator. LPA toured inside and the outside play area. The following was observed and/or noted: Days and hours of operation are Monday -Friday; 7:45am \342\200\223 2:45pm. A review of staff and children's records were conducted as part of this evaluation. \302\267 The licensee/director is asked to update the following documents, if applicable, and submit to licensing within 30 days: 1. LIC 500 Personnel Report 2. LIC 610 Emergency & Disaster Plan 3. Parent Handbook/ Program Curriculum/Admission policies and procedures/ fee schedule (only if changes have been made) 4. LIC 309 Administrative Organization (only if changes have been made) 5. LIC 308 Designation of Administrative Responsibility (only if changes have been made) \302\267 The following items have been posted and are current: License; Emergency Disaster Plan (LIC610); Facility sketch; Earthquake Preparedness Checklist (LIC9148); Parent\342\200\231s Rights Poster (PUB393); Personal Rights (LIC613A); Child Car Seat Law, and Menu. LPA went over with staff to ensure all items are in a visible location as some documents had items covering them making them not visible. \302\267 The facility is operating within the limits as stated on the license and ratios were being met during this inspection. \302\267 Classrooms are adequately equipped with age and size appropriate furniture and equipment and free of hazards. \302\267 There are no weapons present at the facility as stated by Facility Representative Alexis Bogarin. \302\267 There are no accessible bodies of water present at this time. \302\267 Medications shall be stored where inaccessible to children, currently no medications are being stored. \302\267 Hazards are stored where inaccessible to children which include: Disinfectants, cleaning solutions and other items that are dangerous. \302\267 Poisons and toxins shall be locked. While touring the children\342\200\231s restroom LPA observed a high cabinet with the doors open. Inside the cabinet were bottles of hand sanitizer with a label warning if consumed to contact poison control. LPA informed the facility representative to ensure items are maintained under lock. See Technical Violation. \302\267 All floors shall be clean and safe. \302\267 Bathrooms shall be safe, sanitary and in operating condition. While touring the bathrooms LPA observed both bathrooms had several boxes and storage items inside. LPA went over their observations with the facility representative to ensure the safety of the children. See Technical Violation. \302\267 Playgrounds are enclosed by appropriate fences and free of hazards. \302\267 Outdoor activity areas are supplied with age and size appropriate equipment in good condition. \302\267 Food preparation area shall be clean, free of litter, rubbish and free of rodents and other vermin. Food shall be stored appropriately and protected from contamination. Food is prepared off site and delivered to the facility. \302\267 All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. \302\267 Menus shall be posted at least one week in advance in a place visible by the child\342\200\231s authorized representative, dated and kept on file for 30 days, and made available upon request. While touring LPA observed the current months menu was not posted. LPA addressed it with the lead teacher who updated the menu during the inspection. See Technical Violation. \302\267 Uncontaminated drinking water shall be readily available both indoors and outdoors. Children bring their water bottles from home and there are available water fountains on site both inside and outside the classroom. Additionally, facility has plastic water bottles available if needed. \302\267 The areas around or under high climbing equipment, swings, slides, and similar equipment shall be cushioned with material that absorbs a fall. Playground has rubber turf. \302\267 Sign in/Sign out record was reviewed and meets regulation requirements. \302\267 A staff member is present with current Pediatric CPR/First Aid which expires on 1/9/27 \302\267 A review of children\342\200\231s records was conducted, and records were found to be complete during this inspection. \302\267 Disaster drills to be conducted every six months \342\200\223 last drill conducted on 1/16/26 \302\267 The Licensee was informed of their reporting requirements and is provided with the Regional Office\342\200\231s Unusual Incident Reporting email mailbox: UnusualIncidentReportsDO09@dss.ca.gov \302\267 A review of staff records on 2/2/26 indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. \302\267 The Licensee can submit transfer forms to associate new individuals or to disassociate someone from your facility at: Associations_Disassociations862@dss.ca.gov \302\267 A review of staff records indicates that all staff present meet minimum qualifications for the position for which they were hired. \302\267 CCC completed lead testing on 10/7/23. Currently there are no lead exceedances. - Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP . When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: http://www.ada.gov/childqanda.htm - Facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. - Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform. To receive important licensed-related information to licensed facilities, visit the CCLD Important Information website at: https://www.cdss.ca.gov/inforesources/community-care-licensing/subscribe and select the Child Care option to receive email communication. - Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. - The Duty Officer is available to answer questions Monday \342\200\223 Friday; 8:00am to 5:00pm at: 1-844-LET-US-NO (1-844-538-8766) and/or 951-782-4200 See LIC809-D for cited deficiencies. LPA Mejorado informed facility representative Alexis Bogarin that this report dated 2/2/26 document(s) 1 Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Also, LPA Mejorado informed the facility representative Alexis Bogarin to provide a copy of this licensing report dated 2/2/26 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. During the exit interview, the facility representative Alexis Bogarin, confirmed that there are no Registered Sex Offenders present in the facility and LPA completed the RSO profile in FAS.
