What the inspector wrote

On the date and time listed above, Licensing Program Analyst (LPA) Chase Atherton arrived at the facility for the purpose of a POC (Plan of Correction) visit. LPA was granted access by licensee, Guadalupe Guzman. The facility was visited for an annual inspection on 6/16/2025. During this inspection citations were issued. LPA stated the purpose of the visit and began to tour the home and took a census. LPA collected several POCs during todays visit, but there are still uncleared POCs. Licensee Guadalupe Guzman agrees submit proof of completion of all agreed upon Plan of Corrections by 9/13/2025. No deficiencies were cited during today's inspection. A notice of site visit was given to licensee Guadalupe Guzman and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements will result in an immediate $100 civil penalty. Exit interview conducted and report was reviewed with the licensee Guadalupe Guzman.
