Based on observation, the licensee did not comply with the section cited above. In all but one of the preschool classrooms, chemicals were found behind cabinets that were not kept locked. Any harsh chemicals that mention "poison control" on them are considered poisonous and must be kept behind a key lock. This requirement was not being met as evidenced by record review, the licensee did not comply with the section cited above. In reviewing the list of staff associated to the facility, Cheng Cheng Li was not listed, yet has been employed for some time. This poses an immediate health, safety or personal rights risk to persons in care.A $500.00 civil penalty is also being issued.

Responsibility for Providing Care and Supervision - No child(ren) shall be left without the supervision of a teacher at any time... Supervision shall include visual observation.This requirement has not been met as evidenced by: Based on Interviews and faciity's own admission via UIR, staff were unaware a child was left unattended in the playground area for at least one minute. This poses an immediate health and safety risk to the children in care.

Based on [(observation) , the licensee did not comply with the section cited above in that staff brought out, and left cleaners on children's shelf accessible during napping time which posed a potential health, safety or personal rights risk to persons in care. LPA observed children on cots sleeping however 4 children were awake with direct supervision of a 2nd teacher.

Based on [(observation), the licensee did not comply with the section cited above in that during tour LPA observed and obtained photos of built up debris around and inside toilets, sink areas, on bathroom and classroom floors and empty classroom with debris on floors and in disarray which poses a potential health, safety or personal rights risk to persons in care.
