On 12/17/24 at 11:50 pm, Licensing Program Analyst (LPA) Patricia Berry conducted a subsequent case management incident investigation in response to the receipt of an unusual incident report (UIR) from the facility. The UIR was received by the licensing agency on 11/7/24. LPA toured facility and observed no children present during visit. It was reported a staff inappropriately redirected a child. LPA interviewed all pertinent parties. Based on information gathered, the LPA could not determine if a violation did or did not occur, therefore, there is no violation identified at this time. The facility contacted the parent and Community Care Licensing immediately after the incident occurred. An exit interview was conducted and a copy of this report, appeal rights and notice of site visit were provided to the assistant director, Notice of Site Visit must be posted for 30 days.
