What the inspector wrote

On 5/1/25 at 8:20 am Licensing Program Analyst (LPA) Patricia Berry conducted a case management visit in response to the receipt of an unusual incident report (UIR) from the facility. The UIR was received by the licensing agency on 4/24/25. It indicates that a medication was found accessible to a child in care. LPA interviewed all pertinent parties and based on the information gathered, the facility admitted culpability a medication was found accessible to a child in care, therefore the following violation has been cited. The director notified the parent and Community Care Licensing immediately. See LIC809D for cited deficiency of the California Code of Regulations, Title 22, Div. 12. LPA informed director, that Type A citation must be reported to all authorized representatives/guardians of all children currently enrolled by the next business day, or the next day children are in care, and all newly enrolled children for the next 12 months from the date of citation. The signed Acknowledgement of Receipt LIC 9224, must be placed in child’s file for verification. An exit interview was conducted with director. During the exit interview, appeal rights were discussed/provided, Notice of Site form and LIC 9224 Acknowledgment of Receipt, and a copy of this report was provided. Notice of Site visit must be posted for 30 days.
