On this date and time, Licensing Program Analyst (LPA) Aman Sharma arrived at the facility on a Case Management investigation to follow-up on an unusual incident report submitted by the facility on 05/15/2023. LPA took census, met with the center director - Kristen Garnatz, discussed the reported incident, and toured the facility (specifically where the reported incident took place). During todays inspection, the alleged teacher who was mentioned in the incident was not available for interview. One of the subject child(ren) involved in the incident was also not available for interview. Staff and children records was reviewed. The LPA will conclude this investigation at a later time. More time is needed to interview the individuals involved in the alleged incident. An exit interview was conducted, a copy of this report was provided to the center director, Kristen Garnatz .
