On the date and time listed, Licensing Program Analyst (LPA) Eric Ramos arrived at the facility for the purpose of concluding a Case Management inspection that was initiated on 12/19/2025. LPA was granted entry into the facility by Kristen Garnatz. LPA explained the purpose of today's inspection, completed a facility tour, obtained census and interviewed pertinent parties. On 12/03/2025, it was reported that a child was hit by a block by a staff member on accident during clean up time. Information obtained disclosed that there was no malicious intent to injure the child. The child did not receive medical attention due to the incident. Based on information obtained during the investigation, it has been determined there is no violations of Title 22 Regulations pertaining to the reported incident at this time. No deficiency cited at this time. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with Director Kristen Garnatz.
