What the inspector wrote

On 03/14/2024, at 12:30 PM, Licensing Program Analyst (LPA) Raymond Moorehead conducted a case management inspection with Juliana Tahan, Director. A case management inspection is being conducted in response to the receipt of an Unusual Incident Report (UIR) phone call from the facility. The UIR phone call was received by the licensing agency on 03/12/2024 as the incident was self-reported. During today's visit, LPA investigated the incident, Director was interviewed, a tour/census was taken, and the following was disclosed. The UIR documented an incident where the facility's main water pipe reportedly bursted. The main water line is located outside in the front of the building. Further, Director stated that on 03/12/2024 they noticed that water was spurting out of the main line. It was also stated that the water partially flooded the front of the building where the parking lot is located. However, the water did not reach inside of the facility. All children were sent home on 03/12/2024 as a safety precaution, and no children or staff members were harmed during or after the incident. The facility was closed only for the day of (03/12/2024) and has returned to normal operations starting on 03/13/2024. The main water pipe was repaired on 03/12/2024. On 03/12/2024, the City of Redlands assisted by temporarily shutting off the water to allow for repair. LPA observed the area in which the main water pipe bursted. The area was located outside of the facility, and the inside of the facility was not impacted. Therefore, no safety hazards were observed at this time. Based on the information obtained, there appeared to be no violations of Title 22 Regulations pertaining to the reported incident at this time. LPA conducted an exit interview with Juliana Tahan, Director and provided a copy of this report. A Notice of Site Visit was issued and must remain posted for the next 30 days.
