What the inspector wrote

On 05/30/2023 at 3:00 PM A required \342\200\223 1 year inspection is being conducted as part of a compliance review. Licensing Program Analyst (LPA) Susan Brewer, was greeted by Director Juliana Tahan, granted entry to tour the school-age center, inside and out. The following was observed and discussed. The licensee was observed to use the school age classroom for preschool children, during school hours when school age children are not present. The licensee was informed of the requirement to submit a waiver request for daily use of the classroom. The inspection consisted of reviews of the following domains: Food Service Reporting Requirements Physical Plant, Care and Supervision Children Records, Staff Records Staffing Ratio and Capacity, Personal Rights. The inspection found the facility to be in compliance in these domains, except as noted on the LIC809-D. Days and Hours of Operation: Monday through Friday, 6:30 AM to 7:15 AM and 12:10 PM to 6:30 PM A review of the staff records and children's records were conducted as part of this evaluation. The licensee is asked to update the following documents and submit to licensing within 30 days: Only if changes have been made:0 1. LIC 500 Personnel Report 2. LIC 610 Emergency & Disaster Plan 3. Parent Handbook/ Program Curriculum/Admission policies and procedures/ fee schedule (only if changes have been made) 4. LIC 309 Administrative Organization 5. LIC 308 Designation of Administrative Responsibility The following items have been posted and are updated where necessary: - License, Emergency Disaster Plan (LIC610) and Earthquake Preparedness Checklist (LIC9148) - Parent\342\200\231s Rights Poster (PUB393), Personal Rights (LIC613A); Child Car Seat Law, Menu \302\267 The facility is operating within the terms of the license. \302\267 Facility is clean, safe and sanitary and in good repair. \302\267 Ratios were met during this inspection. A census was taken of 27 children in care. \302\267 Appropriate supervision was provided during this inspection. 4 teacher\342\200\231s present and supervising. \302\267 Classrooms are equipped with age appropriate furniture and equipment in good condition \302\267 Classrooms are clean however the restroom in room was not free of hazards. LPA observed a restroom door to be damaged, warped and disconnecting from the hinge. One toilet located in the restroom did not have a cover, exposing the water in the tank. There were no children observed to be in the restroom the time of the inspection. Per director, the restroom is off-limits until the door is fixed and the tank behind the toilet is fixed. \302\267 All materials and surface accessible to children are toxic free. \302\267 No weapons stored at the facility. \302\267 There are no bodies of water present. All wading pools or similar product must be emptied immediately after use and stored in an upright position. \302\267 Medications are stored where inaccessible to children, in the director's office in a locked cabinet. \302\267 Center is equipped to isolate and care for any child who becomes ill during the day. \302\267 Hazards are NOT stored where inaccessible to children which include: disinfectants, cleaning solutions and other items that are dangerous to children, where LPA observed School Age classroom B to have disposable cups, bowls, snacks in the same storage cabinet with a plunger and cleaning solution. The licensee removed the items in the present of the LPA. \302\267 All floors shall be clean and safe. \302\267 Measures are taken to keep the facility free of flies, other insects and rodents \302\267 Toxins were Locked and accessible in an after school classroom. \302\267 Bathrooms were observed to be safe, sanitary and in operating condition, with the exception of the toilet indicated in School age Room B. \302\267 Playgrounds are enclosed by appropriate fences and free of hazards \302\267 Outdoor activity areas are supplied with age and size appropriate equipment in good condition \302\267 Food preparation area is clean and free of vermin. \302\267 Food is NOT stored appropriately and protected from contamination, where LPA observed School Age classroom B to have disposable cups, bowls, snacks in the same storage cabinet with a plunger and cleaning solution. The director removed the items in the presence of the LPA. \302\267 All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair \302\267 Uncontaminated drinking water shall be readily available both indoors and out and is provided by tap water, disposable and individual drinking cups. \302\267 The areas around or under high climbing equipment, swings, slides, and similar equipment shall be cushioned with material that absorbs a fall. Natural grass. \302\267 Menus shall be posted at least one week in advance in a place visible by the child\342\200\231s authorized representative, dated and kept on file for 30 days, and made available upon request. \302\267 Sign in/Sign out record was reviewed and meets regulation requirements. \302\267 A Staff member is present with current Pediatric CPR/First Aid which expires on 04/2025 \302\267 Opening and closing staff member\342\200\231s CPR/First Aid expires on 04/2025 \302\267 Director completed Health and Safety Training on 01/07/2018 \302\267 Staff qualifications and files were reviewed \342\200\223 health screening is on file and all staff meet educational requirements and health requirements for performing assigned tasks. However, one staff member did not have the MMR or Tdap on file and 2 staff members were missing the mandated reporter training certificates Part 1 and Part 2. \302\267 Staff received on the job training for housekeeping, sanitation and universal health precautions \302\267 Each child\342\200\231s file is NOT complete, where 1 child\342\200\231s file reviewed was missing the personal rights and parent\342\200\231s rights forms. \302\267 Licensee was informed of the Department has inspection authority per Health and Safety Codes \302\267 Documentation of Lead Testing is on file: Notification received on 02/21/2023, no lead exceedance. \302\267 A review of staff records on 05/30/2023 indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. The Licensee was informed of their reporting requirements and is provided with the Regional Office\342\200\231s Unusual Incident Reporting email mailbox: UnusualIncidentReportsDO09@dss.ca.gov This facility provides Incidental Medical Services \342\200\223 IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. The director was reminded that all adults 18 and over working at the facility, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. The Licensee can submit transfer forms to associate new individuals or to disassociate someone from your facility at: Associations_Disassociations862@dss.ca.gov for Riverside Regional Office.
