What the inspector wrote

On date and time listed above, Licensing Program Analyst (LPA) Raymond Moorehead arrived at the facility to continue a Case Management inspection, regarding a self-reported incident. The Department received a self reported Unusual Incident Report from the facility regarding an incident where two children allegedly engaged in inappropriate behaviors while on the bus, on the way to the facility, from their elementary school. Throughout the course of the investigation, LPA conducted interviews with pertinent individuals and collected documentation. Due to disenrollments, LPA was unable to conduct child interviews. The subject teacher who was the bus driver was interviewed and reported that they did not observe or witness what was alleged in regards to this incident report. Further, the subject teacher who was the bus driver reported that both children are quick to report things to her, and that no one reported this alleged incident to her at/around the time of the report. It was noted that conflicting statements from both subject children were received regarding the nature of the reported alleged incident. Director stated that both children blamed each other for the alleged incident. Further, it was stated that it was unclear if the alleged incident occurred, and if the alleged incident did occur, it was unclear on which child was the perpetrator. Based on the information obtained through pertinent interviews and documentation, there was not sufficient evidence to determine that a violation of Title 22 regulations regarding Personal Rights occurred. No deficiencies were cited during today\342\200\231s visit. LPA conducted an exit interview with Director Juliana Tahan and provided a copy of this report. A Notice of Site Visit was issued and must remain posted for the next 30 days.
