What the inspector wrote

On date and time listed above, Licensing Program Analysts (LPAs) Chase Atherton and Giselle Carbullido arrived at the facility to conduct a Case Management Visit. LPAs met with facility representative, Crystal Reynolds, toured the facility, and obtained a census. A case management visit is being conducted in response to the receipt of an unusual incident report (UIR) from the facility. The LPAs conducted an initial inspection for this UIR on 5/14/2025. The UIR was received by the licensing agency on 3/5/2025. It indicates that a child's ankle began hurting while in care. Then, three days later, after the child had been absent from the facility, the facility was notified that the child sustained an injury to their ankle at an undetermined time. Based on information gathered, the facility acted appropriately, and no violations have been identified at this time. Facility records were reviewed and pertinent parties were interviewed. It indicates that the facility applied comfort, first aid, and maintained communication with the child's authorized representatives. Additionally, facility completed notification to the department as required by Title 22 Division 12 regulations. An exit interview was conducted and a copy of this report was provided to facility staff. A notice of site visit was given to the facility representative Crystal Reynolds and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
