What the inspector wrote

A case management visit is being conducted in response to the receipt of an unusual incident report (UIR) from the facility. The UIR was received by the licensing agency on 9/18/2025. It indicates that an authorized representative of a child in care reported to the facility that their child, Child 1 (C1), stated that they received a minor scratch from their teacher, Staff 1 (S1). Facility records were reviewed and pertinent parties were interviewed. Based on the available information and information gathered, the facility acted appropriately, and no violations have been identified at this time. The facility provided care to the child with the minor scratch. An exit interview was conducted and a copy of this report was provided to facility representative Crystal Reynolds. A notice of site visit was given to facility representative Crystal Reynolds and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
