On date and time listed above, Licensing Program Analyst (LPA), Giselle Carbullido conducted a Case Management inspection due to unresolved outlets with lead exceedance: Outlets D and E are 2 side by side drinking fountains. LPA took census, and met with facility representative, Meaghan Henriquez to discuss lead testing results and remediation efforts. LPA Carbullido toured the facility to verify remediation and observed outlets D and E accessible but in use for public school not school age afterschool program. During this visit, LPA and facility representative spoke to the principal of the school who extended area to provide water dispenser with bottled water service. Facility representative ordered water dispenser during this visit. During this visit LPA reviewed process for lead testing including documentation required per PIN 21-21.1-CCP. An exit interview was conducted, and appeal rights discussed. LPA Carbullido provided facility representative, Meaghan Henriquez with a copy of this report, appeal rights and notice of site visit. This report must be made available to the public upon request for three years
