What Happened

Complaint investigation regarding hand-foot-mouth disease outbreak notification. Facility notified only classroom 3 parents of HFM cases but failed to notify classrooms 1 and 2. Facility also failed to report to Licensing. Allegation substantiated.

Observations

  • First confirmed HFM case in classroom 1 (09/21/2022)
  • Two additional HFM cases in classroom 3 (09/23/2022)
  • Third HFM case in classroom 3 (09/26/2022)
  • Parents of classroom 3 notified on 09/26/2022
  • Parents of classrooms 1 and 2 NOT notified of potential outbreak
  • Facility failed to report to Licensing (more than 2 cases within 7 days)
  • Type B deficiency cited
