What the inspector wrote

On date and time listed, Licensing Program Analyst (LPA) Chase Atherton arrived at the facility to conduct an case management inspection. LPA toured the facility, inside and out, and took census. A case management visit is being conducted in response to the receipt of an unusual incident report (UIR) from the facility. The UIR was received by the licensing agency on 11/19/2025. It indicates that a parent of a child (Child 1 [C1]) reported that C1 stated that a staff member (Staff 1 [S1]) touched C1's throat and mouth after C1 was throwing things in the classroom. Facility records were reviewed and pertinent parties were interviewed. Based on information gathered, the facility acted appropriately, and no violations have been identified at this time. The facility investigated the report themselves by interviewing multiple staff members and children. The facility also reported the incident to the Department as required by Title 22 regulations. An exit interview was conducted and a copy of this report was provided to facility representative Amy Halit. A notice of site visit was given to facility representative Amy Halit and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
