What the inspector wrote

Licensing Program Analysts(LPAs) Blanca Ruiz and Elyse Jones arrived at the facility to conduct a Case Management inspection for the purpose of addressing separate matters that were discovered during an inspection at the facility. During the inspection LPAs\302\240 conducted a tour of the facility and census were taken. According to Title 22,\302\240 facility is required to provide sufficient staff in order to meet all the children's needs and to provide a safe environment for all the children in care. Facility documentation written by staff, reports child(ren) has/have hitting and scratched other children and staff on multiple occasions. Information received throughout the investigation revealed that facility staff w ere aware of the situation and\302\240 that closer supervision was required to shadow the subject child(ren) to prevent the hitting incidents.\302\240 However, facility failed to rearrange staffing\302\240 and/ or to work a plan to\302\240meet the needs of all the children in the class and to protect the safety of children in care. See LIC 809-D for deficiency cited Exit interview conducted and report\302\240 was reviewed with Director, Jennifer Diehl. A Notice of Site Visit and Type A Deficiency was given and must remain posted on, or immediately adjacent to the interior of the main door for 30 days. Appeal Rights discussed and given to facility representative, along with a copy of this report and LIC 9224 was given to the licensee or facility representative. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Upon receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility for the next 12 months.
