What Happened
Complaint investigation substantiated two violations: (1) Delayed reporting of suspected Hand, Foot, and Mouth Disease outbreak to local health officer; (2) Operating out of mandated staff-to-child ratios for extended periods when children dropped off in succession.
Observations
- Delayed reporting of suspected Hand, Foot, and Mouth Disease outbreak to local health officer (violation of Title 22 immediate reporting requirement)
- Operating out of required staff-to-child ratios for 5+ minutes on multiple occasions
- Two out of three randomly selected dates showed multiple instances of rooms operating out of ratio
