What the inspector wrote

On March 25, 2026, Licensing Program Analyst (LPA), Ali conducted an unannounced complaint 10 day and findings inspection to Kaleidoscope Children’s Center. LPA met with Director (facility representative) who granted access. The purpose of the inspection was to present 10 day and findings from complaint investigation (3/20/2026) regarding the above allegation. LPA discussed the allegation details with Director. LPA observed 13 daycare children, 2 staff, and 1 director present in the facility. During the investigation, LPA conducted interviews with all relevant parties. Based on observations, interviews, record reviews, the evidence revealed inconsistent statements with Allegation #1: Children hit C1 and staff did nothing about it. The allegation could not be corroborated. Although the allegation(s) may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated. An exit interview was conducted, the report was read, and a copy of this report was left with the Director with notice of the site visit and appeal rights. Failure to maintain posting of the Notice of Site Visit for thirty (30) consecutive days will result in a $100 Civil Penalty.
