What the inspector wrote

On Thursday, May 1, 2025, Licensing Program Analysts (LPA) Cha and Braddock met with facility representative, Jonilda Barajas ,. The purpose of the inspection was to conduct an unannounced case management inspection for the self reported Unusual Incident Report (UIR) received at Palmdale Regional Office on 04/24/25. LPA disclosed the purpose of the inspection to the facility representative. When LPA arrived at the facility there were 31 children with nine staff. LPA observed waiver's posted regarding mainstreaming special education children during indoor and outdoor activities and co-mingling preschool and kindergarten children during outside play.The hours of operation for the program is Monday through Friday 8:30AM-11:30AM and 12:30PM- 3:30 PM. During the inspection LPAs obtained a roster of children present on the day of the incident. LPA interviewed staff, children, and parent of C1. LPA completed a safety inspection of the facility at approximately 11:20am. Based on LPA observations and interviews, no further investigation is needed. A Notice of Site Visit was given and must remain posted for 30 days. Exit interview was conducted and the report was reviewed with the facility representative Jonilda Barajas.
