What the inspector wrote

Licensing Program Analysts (LPAs) Susan Brewer and Laura Mejorado, conducted a case management visit in response to a self-reported unusual incident report (UIR) from the facility. The UIR was received by the licensing agency on 03/26/2026. It indicates that a subject staff left a subject child unattended in a classroom. The LPAs were greeted and granted entry to tour the facility inside and out. The LPAs took a census of 12 children present supervised by 3 staff. The LPAs met with the director, made observations, reviewed facility records and conducted interviews with pertinent parties. Based on the information gathered, the following violations have been identified. It was determined that an absence of supervision took place on 03/26/2026 at approximately 4:15 PM, when a subject child was left unattended in a classroom for approximately 3 minutes during a staff rotation, and due to a lack of communication between staff during the transition phase. The licensee conducted an internal review of the incident and self- reported the incident to the department on the day the incident occurred. LPAs Susan Brewer and Laura Mejorado informed Director Melissa Speakman that this report dated 04/13/2026 documents a Type A violation of the California Code of Regulations, Title 22, Div. 12, 101429(a)(1) Responsibility for Providing Care and Supervision for Infants, which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. See LIC809C Page 2 Also, LPAs Susan Brewer and Laura Mejorado informed the Director Melissa Speakman to provide a copy of this licensing report dated 04/13/2026 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. If a Civil Penalty has been assessed during this inspection. Payment is due when billed and the check(s) or money orders shall be made payable to the “California Department of Social Services”. YOU WILL RECEIVE AN INVOICE IN THE MAIL. DO NOT SEND MONEY UNTIL YOU RECEIVE YOUR INVOICE. DO NOT SEND CASH. A Civil Penalty was issued in the amount of $500.00, on today's date. An exit interview was conducted, appeal rights discussed, and a copy of this report was provided to the director Melissa Speakman. The LPAs provided the licensee was a Notice of Site Visit. The LPAs observed the licensee post the Notice of Site Visit prior to exiting the facility.
