101416.5 Staff-Infant Ratio (d) There shall be one teacher to every 12 sleeping infants provided the remaining staff necessary to meet the ratios specified in (b) above are immediately available at the center.This requirment is not met as evidenced by: Based on observation there were 7 sleeping infants and 5 awake infants with only 1 staff providing care. Of the 5 awake infants, 2 were seen off their cots directly next to the staff, while 3 were sitting up on their cots. This poses/posed an immediate helath, safety, and/or personal rights risk to persons in care.

This is an amended report delivered on 8/7/2025.101238 Buildings and Grounds (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors.This requirement is not met as evidenced by: Based on observation and interview, the facility had debris on the floor of the infant classroom including food, dust, discolored marks on the floor, and dark or discolored marks on a significant portion of the walls in the infant’s reach. Infants were observed crawling on the floor and putting their hands and feet in their mouth. Based on this information this poses/posed an immediate health, safety, and/or personal rights risk to persons in care.
