Inspector’s narrative

What the inspector wrote

Licensing Program Analysts (LPAs) Chase Atherton and Aman Lama arrived at the facility to conduct an unrelated inspection. During the tour of the facility during that inspection, LPAs observed 7 sleeping infants and 5 awake infants with only 1 staff providing care. Of the 5 awake infants, 2 were seen off their cots directly next to the staff, while 3 were sitting up on their cots. This does not comply with Title 22 regulations in that there shall be 1 teacher to every 12 sleeping infants. As not all infants were sleeping the facility should have been following the ratio of one teacher to every 4 infants in attendance. See LIC809D for deficiency cited. LPA Atherton informed Director Tracy Bierman that this report dated 4/24/2025 document(s) 1 Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Atherton informed the Director Tracy Bierman to provide a copy of this licensing report dated 4/24/2025 that documents any Type A citation(s) to parents of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Appeal Rights issued and discussed with facility representative and their signature on this form acknowledges receipt of these rights. Exit interview conducted and report was reviewed with the Director Tracy Bierman. A notice of site visit was given to Director Tracy Bierman and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. This report must be made available to the public for 3 years. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

    This is an amended report delivered on 8/7/2025.101238 Buildings and Grounds (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors.This requirement is not met as evidenced by: Based on observation and interview, the facility had debris on the floor of the infant classroom including food, dust, discolored marks on the floor, and dark or discolored marks on a significant portion of the walls in the infant’s reach. Infants were observed crawling on the floor and putting their hands and feet in their mouth. Based on this information this poses/posed an immediate health, safety, and/or personal rights risk to persons in care.
