What Happened
Annual inspection found Type B deficiency: staff 1 completed wrong mandated reporter training course. Facility maintenance issues identified and corrected during visit: displaced cushioning material under climbing structure replaced immediately, damaged tricycle immediately removed to inaccessible area. Overall facility maintains safe environment and proper compliance.
