What the inspector wrote

Licensing Program Analyst (LPA) Maddox met with Lead Teacher, Patricia Vasquez for the purpose of conducting an unannounced Req 1 yr inspection. Present today were 20 children and 3 staff persons. This preschool program operates within modular # PS22 on the campus of Vermont Elementary School. This modular has a main classroom area, boys and girls bathrooms; and an office. THE PROGRAM WILL OPERATE TWO HALF DAY SESSIONS/ Mon-Fri from 8:30 AM TO 11:30AM AND 12:30PM TO 3:30PM. **LPA observed age appropriate furniture, equipment , toys and materials. The classrooms were observed to be clean and safe and free of any Health or safety hazards. Telephone service was verified as well as adequate heating, lighting, and ventilation. Children's belongings are kept in cubbies along the wall as you enter classrooms. There is a drinking fountain inside the classroom for drinking water. Lead testing has been completed, water samples were within normal range. ** The children's bathrooms (boys and girls) are located within the classroom, each bathroom has 1 toilet and 1 sink. LPA observed the bathrooms to be clean and sanitary, with soap, toilet paper and paper towels readily available. Toilets and sinks are functioning properly and age appropriate. **Children do no nap at this center, half day programs only. ** Outdoor play equipment was inspected for health, safety, good repair and age appropriateness. Center utilizes rubber matting under jungle gym climbing structures for cushioning material and there is sufficient and adequate shade available. Large play equipment and climbing structure is securely anchored. The area was observed to be free of debris. Staff bring water outside with disposable cups. There are no bodies of water observed on the premises. The playground is enclosed by a fence to protect children and to keep them in the outdoor activity area. * *Center provides breakfast in the AM class and lunch in the PM class - Menus posted. - Food is delivered daily from the cafeteria Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children were stored and inaccessible to children **The Parent Board located within the classrooms has contains required licensing forms. A sampling of Children's records were reviewed as part of this inspection. Staff are certified in Pediatric CPR and First Aid (exp 4/4/2025), children files reviewed today were complete including current/required immunization's. Staff files are maintained at the SAN BERNARDINO CITY UNIFIED SCHOOL DISTRICT OFFICE Center utilizes M-Sign to record the Sign in and out of each child. LPA observed smoke detector and carbon monoxide detector in place. Fire Drill/Earthquake log is posted. As a result of this inspection, no citations were issued. Exit interview conducted and a copy of this report was left at the facility. A copy of this r eport must be made available to the public for 3 years.
