What the inspector wrote

During staff interviews, staff members were able to identify children that display aggressive behaviors. Staff was able to disclose and identify methods of preventative and correction action for aggressive behaviors that take place at the facility. Further, Director was able to provide LPA and LPM with documentation of a behavior report and plan. Director also mentioned that the facility has used outside services, in order to help with the display of aggressive behaviors. During other pertinent interviews LPA and LPM received conflicting statements. Some of the pertinent interviews stated there is a child that hurts other children. At the same time, some of the pertinent interviews disclosed that the child mentioned does not hurt other children. Further, those who stated that there was a child who hurts other children stated that in the event of the incident, teachers would intervene and talk with the child if they hurt a friend. The Department was unable to determine if all incidents were observed by staff members or not. This agency has investigated the complaint regarding the above allegation. Based on the interviews conducted and documentation collected, the allegation is UNSUBSTANTIATED. A finding of unsubstantiated means, although the allegations may have happened, or are valid, there is not a preponderance of the evidence to prove the allegations occurred. No deficiencies were cited during this inspection. A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with Deanna Brown, Director.

    Based on observation, the licensee did not comply with the section cited above in which poses a potential personal rights risk to persons in care. During inspection of medication that is stored in the classrooms, LPA and LPM observed medication for C7 to not have a prescription on file.
