What the inspector wrote

On March 28, 2023, Licensing Program Analyst (LPA) Babatunde Ibitoye met with Site Supervisor Patricia Rosales to conduct an unannounced case management inspection. The purpose of the case management was to follow up on a self reported unusual incident report (UIR) submitted to the Department on March 9, 2023. Description of incident: On 03/08/2023 Site supervisor was notified by Behavior Specialist B#1 regarding parent #1 complaint for child care career worker CCC #1 in classroom #90.Parent #1 stated,that on 3/7/23 upon pick-up time CCC worker #1 grabbed Child #1 by his arm and told C #1 ''Didn't I told you to stop hitting me''.After C#1 tapped CCC worker #1 to show her some blocks. Present during the time of this inspection is the site supervisor, teacher #1,Aide #1 The copy of the facility roster, sign-in and out sheet for the incident day was collected. Further investigation is needed, An exit interview was conducted, and a copy of this report was read and provided to the site supervisor Patricia Rosales along with a Notice of the Site Visit
