On 6/19/25 Licensing Program Analyst (LPA) Patricia Berry Conducted a case management visit is being conducted in response to the receipt of an unusual incident report (UIR) from the facility. The UIR was received by the licensing agency on 6/5/25. It indicates that a child was injured requiring stiches. c1-Malik Calloway LPA interviewed staff. Staff stated the c1 put their finger in the bottom of the trash can pedal and c1's ring got stuck on the pedal. Staff stated they noticed c1 pulling out their finger and the ring got caught on the trash can pedal and staff stated they helped release c1's finger and staff noticed the ring cut their finger. Based on the information gathered, the facility acted appropriately, and no violations have been identified. Director applied first-aid, called the parent and notified Community Care Licensing immediately. An exit interview was conducted, and a copy of this report was provided, appeal rights and notice of site visit. Notice of Site Visit issued and must be posted for 30 days.
