Licensing Program Analyst (LPA) Laura Mejorado arrived at the facility, for the purpose of conducting a case management follow-up visit; regarding an unusual incident report that was submitted by the facility. LPA met with Director Luz Gonzalez, toured the facility and discussed the reported incident. On 1/19/23 upon arrival staff noticed water flooding around building #3.
