What the inspector wrote

On July 16, 2024, Licensing Program Analyst (LPA) Taityana Benson arrived at the facility to conclude the investigation in a separate matter, a previous inspection was conducted on May 09, 2024. LPA met with Arlene Gormley, Master Teacher at arrival and conducted a tour of the facility inside and outside. At 9:22 a.m., Wendy Leath, Director arrived to the facility. It was discovered during record review, the facility did not notify the department via telephone or fax, by the next business day and during normal working hours regarding an alleged incident involving staff-children interaction, that took place on 05/02/2024. The facility was made aware of the alleged incident on 05/02/2024 during the morning hours. Furthermore, the facility did not inform their assigned Licensing Program Analyst of the alleged incident that occurred on 05/02/2024. Lastly, the facility submitted a written report via Unusual Incident/Injury Report (LIC624) to the department on 05/06/2024 based on subsequent events and information provided to the facility on 05/03/2024. See LIC809-D for deficiency cited A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. An exit interview was conducted, and report was reviewed with Wendy Leath, Director.
