Inspector’s narrative

What the inspector wrote

Licensing Program Analyst (LPA) Steven Montoya met with facility Director, Carmen Rodriguez for the purpose of an Annual/Random inspection of the school age component. There were 15 school age children present upon arrival. The center consists of 3 classrooms. This is a combination center which includes day care center (364813683) and infant (364844285) components. The center's hours of operation are: Monday thru Friday 6:00 am to 6:00 pm. During this inspection, LPA toured the facility, observed equipment/supplies and reviewed a sample of children and staff files. Incidental Medical Services (IMS) were discussed. The center currently has no children in the center requiring IMS. Furniture and equipment were inspected for age appropriateness, good repair, cleanliness and safety. Telephone service was verified. Heating, lighting, and ventilation are adequate. LPA observed age appropriate toys and materials. Fire extinguisher, smoke detector and carbon monoxide detector were observed to be in operable condition. There is 1 bathroom with 2 toilets /2 sinks inside and another sink located inside of the classroom. LPA observed soap, toilet paper and paper towels readily available. Staff restroom is in the hallway next to the director's office. Current Pediatric CPR and First Aid certificates were observed as well as Mandated Reporter Certificate for Child Care. Director is aware Mandated Reporter training must be taken every 2 years. Outdoor play equipment was inspected for health, safety, cushioning material, good repair and age appropriateness. There are 3 separate gated playgrounds (1 for each component). The preschool playground is in the back of the center. There is a large play structure, securely anchored with rubber underneath for cushioning. During this investigation, a bee hive was discovered in the school age play area inside of a tree. Director states she will take measures to have the bee hive removed before allowing children to play outside. There is an area for shade and rest. Drinking water is available in the form of a drinking fountain. Play area was inspected for hazards and inaccessibility to bodies of water. Children are inspected for illnesses as they arrive. A review of medication policy indicated that prescription medication is administered and only with parent's written permission. The director or lead teacher administers medication and documents the dosage, date and time onto a log. Medication is brought and taken home by the parent daily or retained in a lock box, properly labeled and stored in its original container. Medications are stored in the locked emergency cabinet cabinet closest to kitchen. There is a separate area for isolation and care of ill children in the director's office. A separate cot with fresh linens available for each ill child. There is a clean, fully equipped kitchen with refrigerator/freezer, oven, microwave and sink. The facility provides breakfast, lunch and snacks. Weekly menus are posted. Allergy lists were observed posted on the refrigerator. LPA observed an appropriate amount of food and snacks in the pantry. The chemicals are kept separate from the food key-locked above the washer/dryer. Teacher/child ratios were observed. Fire/earthquake drills are current. Current children's roster is maintained. Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department.
