During interviews with staff, there was no indication that the Director had taken the child into the director's office on 09/17/2021. Staff denies witnessing the director yelling at or mistreating children. Staff indicated that the Director is good with the children. On occasion, Director will remove the child from the classroom per the teacher's request and take the child to the Director's office to "cool down" for a short time and then take the child back to the classroom. Director indicated that the office door is never shut when a child is in the office. Director denies the allegations regarding the injuries to the child. Staff interviews also indicated that the teacher in the child's classroom, would speak loudly to the children at times and some of the children felt intimidated by the teacher. LPA determined there is conflicting information regarding a Day care staff causing injuries to child while in care. There were no eye witnesses to the alleged incidents reported and the child in question and other pertinent sources were not able to participate in the investigation. Although the allegations may have happened or are valid, there is no preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are deemed UNSUBSTANTIATED at this time. No deficiencies were cited. An exit interview was conducted, and a copy of this report and a Notice of Site VIsit (required to be posted for the next 30 days) was provided to the Director Laura O'Brien on 12/07/2021. LPA verified the Notice of Site Visit was posted in a prominent location before leaving the facility.
