What does Type A vs Type B mean?
Type A. Serious citation. Imminent or substantial risk to children. The regulator requires corrective action immediately and may impose a civil penalty.
Type B. Lower-severity citation. Corrective action required, no imminent risk. The regulator monitors compliance on the next visit.

(g) Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children shall be stored where inaccessible to children.(1) Storage areas for poisons shall be locked. This was not met as evidenced by: During a tour of the facility, although most classrooms had chemicals stored behind a key lock, Room 4 and the restroom between rooms 1&2 had cleaning supplies that were poisonous and not behind a key lock. This poses a potential health and safety risk to the children in care.

FIXTURES, FURNITURE, EQUIPMENT AND SUPPLIES
(n) Furniture and equipment shall be maintained in good condition, free of sharp, loose or pointed parts. This was not met as evidenced by: During a visit to the facility on 07/23/2025, LPA observed 3 cracks and pieces missing from a bench in the outdoor play area. LPA sat at the table during children's outdoor playtime today and saw the same exact cracks still there. Children were seen playing around the cracked bench. This was addressed under a Technical Violation (TV), but is now being cited as a Type A citation. This poses an immediate health and safety risk to the children in care.

This was not met as evidenced by: Day care does not have a qualified director, no did they report to the department within 10 days of a change of a child care center director or designee(s). This poses a potential health and safety risk to persons in care.
