What the inspector wrote

On July 22, 2025, at 1:00 pm., Licensing Program Analyst (LPA) Zirbes met with Facility Representative, Margaret Quintana to conduct an unannounced case management inspection. The purpose of the case management was to follow up on a self-reported unusual incident report (UIR) submitted to the Department on July 16, 2025. The unusual incident report was regarding a potential supervision violation which occurred on July 14, 2025, involving child 1 (C1). Upon LPAs arrival today, there were 16 preschool age children, two teachers, and additional administrative/support staff on site. During this inspection, interviews were conducted with children, staff and parents. In addition, LPA reviewed child files and conducted a safety inspection on this day. LPA did not observe safety hazards during the inspection. Based on interviews and record review, the Department determined that on July 14, 2025, at an unknown time between 08:45am and 12:30pm, C1 received approximately five unexplained bites marks on the left and right forearms while in attendance at the Center. Confidential interviews did not provide additional details regarding the incident involving C1. Facility representative Quintana confirmed C1 received the unexplained bite marks while in care at the Center on July 14, 2025. Over the course of investigating the case management LPA reviewed photographic evidence of the injuries C1 received. A deficiency is being cited based on interviews, and record review in accordance with the California Code of Regulations, Title 22, see LIC 809D. An exit interview was conducted, where the plan of correction was reviewed and developed with facility representative, Margaret Quintana. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted where this report and appeal rights were reviewed with the facility representative, Margaret Quintana.
