What the inspector wrote

On January 16, 2026, at 11:00am, Licensing Program Analyst (LPA) Zirbes conducted a case management deficiencies inspection to address information received regarding Mc Donald Learning Center. LPA met with facility representative Margaret Quintana. Present during today's inspection were 20 preschool children. Based on information obtained through interviews, the Department discovered a reportable incident occurred on an unknown date in October 2025. Staff interviews confirmed that in October 2025, staff 1 (S1) left child 1 (C1) alone and unsupervised on the horse playground. Interviews reported that S1's class was engaged in outdoor play on the horse playground. During the transition from outdoor play to coming back inside the classroom, C1 was left alone and unsupervised in the enclosed horse playground area. Conflicting information was received regarding how long C1 was left alone and unattended. The time frame ranged from 3-5 minutes to 30 minutes. S1 confirmed C1 was left unattended on the playground due to hiding under the climbing equipment. S1 did not realize C1 was left on the playground until S1 was notified by another staff member. A community member who was attending to the neighboring horses came to the Centers front office and informed Staff 2 that C1 was on the playground alone. Per LPA review of the Departments records, this incident was not reported to the Department. Facility Representative Quintana confirmed the incident was not reported to the Department. C1 authorized representative was informed of the incident on December 20, 2025. Based on record review and interviews, on an unknown date in October 2025, C1 was left alone and unsupervised on the horse playground, this was an unusual incident that potentially threatened the emotional health and safety of C1 and therefore this was a reportable incident. A type B citation for regulation 101229 (a)(1) Responsibility for Providing Care and Supervision and one type B citation for regulation 101212 (d)(1)(C) Reporting Requirements is being issued as this was a potential risk to the children in care. An exit interview was conducted with facility representative, Lilia Ross. Appeal rights and a copy of the report were provided to facility representative, Lilia Ross.
