Inspector’s narrative

What the inspector wrote

On this date and time, Licensing Program Analyst (LPA) Laura Mejorado arrived at the facility to conduct a Case Management inspection for the purpose of addressing separate matters that were discovered during an inspection at the facility. During the inspection LPA conducted a tour of the facility and census was taken. LPA reviewed records and conducted interviews with pertinent parties. While conducting interviews it was disclosed there have been numerous biting incidents in the toddler classroom. LPA reviewed documentation which revealed that between April 2025 \342\200\223 September 2025 there were at least 11 biting incidents between the toddlers. Noting one child was bitten two times on the same day with the incidents occurring within three hours of each other. It was disclosed there are no specific biters in the classroom and the incidents have been random. Per interviews the facility biting policy states if the same child bites multiple times they are placed on a strategy plan called My Path. My Path provides staff with strategies on how to work with the child along with strategies for the parents to work with the child at home. Currently there are no children with a My Path plan. Per the parent handbook at the first sight of biting, staff evaluate the environment and if warranted adjustments are made. At this time, based on the information obtained nothing substantial has been implemented, to curve the biting or to ensure that all children\342\200\231s Personal Rights are not being violated. See LIC9099-D for the deficiencies cited. LPA Mejorado informed Director Debra Rivas that this report dated 10/30/25 document(s) one Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Also, LPA Mejorado informed the Director Debra Rivas to provide a copy of this licensing report dated 10/30/25 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview conducted and report was reviewed with Director Debra Rivas.
